1. Enter Your Patient Data

  • Select 5 retrospective ACS patient charts
    • We recommend a mix of STEMI and UA/NSTEMI patients
  • Complete a data collection form for each ACS chart pulled
  • Submit your ACS patient data online, by mail, or by fax:
    • On the My Performance page of this Web site
    • Attach a brief practice questionnaire and mail to Med-IQ Concierge Professional, 5523 Research Park Drive, Suite 210, Baltimore, MD 21228
    • Fax with a brief practice questionnaire to 866 858 7434 (toll-free)

Note:
Your ACS patient data is safe. We do not ask you to share any patient identifying information that would cause you to violate HIPAA privacy guidelines.

All information submitted is confidential. Data may be presented anonymously and in aggregate, but no individual information will be shared externally.

Remember: This performance improvement initiative is meant to help you assess and improve your practice. You will not be graded, and your individual performance will not be shared. When completing the data collection forms, please select “Yes” to an item only if it is documented in your patient charts.

2. Compare Your Patient Data with National Benchmarks,
Performance Guidelines, and Data Submitted by Colleagues

  • Review your My Performance page to find automatically generated reports summarizing how your data compares to national benchmarks, performance guidelines, and data submitted by your peers
  • Review the results of your reports and select one or more areas for improvement

Note:
Your reports will generate immediately if you enter your ACS patient data online.

If you submit your ACS patient data by mail or fax, your reports will appear on the My Performance page two business days after it is received by Med-IQ.

If you do not have time to review your reports online, ask your concierge professional to e-mail or fax your reports to you.

Stage A Complete: Print Your 5-Credit Certificate

Accreditation/Designation Statements: Med-IQ is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Med-IQ designates this activity for a maximum of 20.0 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
